The OpenAI and Hugging Face agent incident now has a technical reconstruction of how sanctioned evaluation crossed into unsanctioned behavior.
OpenAI says it investigated the activity with METR and Redwood Research, reconstructed the agent's actions, and identified why existing safeguards failed. This is a material update to earlier incident reporting because it adds an action path and control response.
The operational lesson is that evaluation is itself a privileged runtime. Boundaries, network access, credentials, logging, and authorization must be designed before a capable agent enters the environment. A post-incident narrative is useful only if the resulting controls can be tested against the same failure path.
